A complete application packet for the Bachelor of Health Science (BHS) program includes:
Information sent to Armstrong's Admission's Office:

• Armstrong Undergraduate Application
• Proof of Immunization
• High School and College Transcripts
• Official Copies of SAT or ISAT Scores

Information sent to the Health Sciences Department:

• Bachelor of Health Science application
• High School and College Transcripts
• Letter of Intent

Gerontology Certificate Application

Students interested in the Gerontology Certificate program must complete:

• All application forms required for admission to Armstrong Atlantic State University:
  • Armstrong Undergraduate Application
  • Armstrong Graduate application
• Gerontology Certificate application (sent to the Department of Health Sciences)
• Letter of Intent
• Interview with the Gerontology Certificate Program Coordinator

Strength and Conditioning Certificate Application

Students interested in the Strength and Conditioning Certificate program must complete:

• Armstrong Undergraduate Application
• Armstrong Graduate application
• Strength and Conditioning Certificate application (contact the Program Coordinator).

MHSA Application

All applicants must apply through Graduate Studies. For more information about the application process and application forms, contact:

MPH Application

All applicants must apply through Graduate Studies. For more information about the application process and application forms, contact:

A complete application packet for the Master of Public Health (MPH) program includes:

• Application
• $30 non-refundable application fee
• Immunization form
• Two (2) Official Transcripts of all post-secondary education
• Letter of Intent
• Two (2) completed Reference Forms
• Completed Work Experience Form
• Official GRE or MAT scores
