Google Group Account Request Form
Note: This is for a Google Group Account. It is the responsibility of the Department Head/Director to decide and manage who has access to this account. More than one person can be assigned as owners/moderators and use this account.
Please provide a first and second choice for the account usernames. The email address format will be email@example.com. You will be notified by phone or email when your account information is ready for pick up.
I verify that the above information is correct. The undersigned applicant hereby requests the account creation on the Departmental email account listed above. Your eSignature (and submission) certifies that you are aware of and will comply with the conditions of issuance of this account as set forth in the Armstrong State University Responsible Use of Technology Resources Policy and all local, state, and federal laws regarding computer use. As the primary Google Group Account owner or moderator, I agree to notify my supervisor or director that the group should be re-assigned, if you either change positions or leave Armstrong and will not continue to administer the group. The Georgia Computer Systems Protection Act is incorporated herein by reference. If you would like copies of these policies or laws, please contact IT Services. I understand that this account is subject to termination without notice should I violate this agreement in any way.
