Forms for Faculty and Staff
Please print the forms and complete. Submit through campus mail to IT Services with the appropriate signatures to be processed. Once the accounts have been created you will be notified by phone or email. Account pickup will occur in the MCC Annex Building at the IT Services Helpdesk counter. To sign for username and password information, please have PirateCard or other photo identification.

AASU Employee Technology Acquisition Form
The AASU Employee Technology Acquisition form is designed for a supervisor and/or Human Resources to request the technology needs for a new AASU employee. Current employees may also use this form to request additional technology resources that were not acquired prior to their arrival on campus.
To ensure new employees have the technology resources needed for employment on the first day, this form must be submitted to IT Services two weeks prior to the start date. If a new computer acquisition is required, a minimum of four weeks is required so that the computer can be purchased, received, and set up for the new employee.
NOTE: Full time and part time faculty and staff are eligible to access email and Novell services. Novell Services provide access to shared drives and network printers. Access to these services can be requested through the AASU Employee Technology Acquisition form.
Banner Account Request
Department Heads, Administrative Assistants, and Registrar and Financial Aid Office are typical users of this application.
PeopleSoft Financials Security Request Form
Use this form for new accounts and security changes for business office staff.
PeopleSoft Workflow Roles Request Form
Use this form for security roles needed to submit cash advances, eProcurement requisitions, or to become a department approver for Expenses or eProcurement.
Departmental Email Account
A department may request a departmental account to receive email that does not need to be directed to an individual.
Lab Access Account
Faculty and staff are required to have a seperate account to access lab computers on campus. For security purposes, all computers on campus require a login.
For faculty/staff to use computers other than the one designated for their use (such as lab and Library computers), a special login is required. Example Faculty Lab Account: .melvinsa-lab.Guest.AASU
Students are required to use their Pirates’ Cove ID and password information to access the computers on campus.

To begin this process, please contact Human Resources and request a name change. Then complete this form and submit it to Human Resources with proof of legal name change.
